Description:
This simple PHP MySQL system has quit working. My host says it may be caused from a PHP upgrade. I need to have it fixed ASAP as customers are on hold. (I can't find the original programmer.)

The popup generated here has custom imbedded affiliate links. More info on how the program works can be supplied if needed.

Until the host made a few changes in the DNS Zones I was getting this error when the form was filled out:

I also need a couple of simple popup scripts written that will work in conjunction with this program. Specifically, one that will launch the popup only once per browser session.

Deliverables:
1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done. Existing program must be repaired to full working condition.
2) Installation of the software on the platform(s) specified in this bid request.
3) Complete ownership and distribution copyrights to all work purchased.
Platform:
Program uses PHP with MySQL database on Linux machine.

Deadline legal notes: All times are expressed in the time zone of the site EDT (UT - 5). If the buyer omitted a time, then the deadline is 11:59:59 PM EDT on the indicated date.
Remember that contacting the other party outside of the site (by email, phone, etc.) on all business projects < $500 (before the buyer's money is escrowed) is a violation of both the software buyer and seller agreements.
We monitor all site activity for such violations and can instantly expel transgressers on the spot, so we thank you in advance for your cooperation.
