Description:
I need an ASP script written for a doctor/dentist web site.
I want the receptionist to enter the patients name + email + date/time of appointment through a web browser (into a database) and have an automatic email appointment reminder sent to the patient a few days prior to their appointment.
I will need the patient to respond to the email and have the response noted in the database so the receptionist knows that the patient has received the appointment notification email.
The script should have the following information entered by the receptionist-
Multi-doctor selector - In case she works for 2 doctors (drop down menu would work)
Ability to set the number of reminders days before the appointment date that the patient will recieve the automatic email.
Patient name - email address - appointment time and date
In the email notice the patient should be given the choice of responding that they will make the appointment or need to cancel thier appointment.
Warning message written to the database or emailed to the receptionist if patient does not respond to the email notice.
Would be ideal for the receptionist to open the daily appointment schedule to see who has not replied to the appointment email.
Appointment notice email should contain the following -
patient's name - email address - appointment time and date.
Email should contain two buttons
"click here" - if you can make your appointment
"click here" - if you need to cancel your appointment
Deliverables:
Complete and fully-functional working program(s) in executable form as well as complete source code of all work done.
Complete copyrights to all work purchased.

Deadline legal notes: All times are expressed in the time zone of the site EDT (UT - 5). If the buyer omitted a time, then the deadline is 11:59:59 PM EDT on the indicated date.
Remember that contacting the other party outside of the site (by email, phone, etc.) on all business projects < $500 (before the buyer's money is escrowed) is a violation of both the software buyer and seller agreements.
We monitor all site activity for such violations and can instantly expel transgressers on the spot, so we thank you in advance for your cooperation.
